• A no show charge of $500 (5 business day notice required) will be assessed for failure to appear (patient or interpreter).
RECORDS:
• In order to provide you with a timely report, records (including prior MRI/CT scan/films) need to be sent 4 weeks prior to the appointment.
•Records are an integral part of the evaluation. If records are not received at least 72 hours prior to the appointment, the appointment may be rescheduled and a no show fee charged.
• $15 will be charged for returned medical records or send a self addressed postage paid envelope.
